US Chemical Safety Board Pins 2024 Fatal Deer Park Refinery Accident on Pemex's Safety Lapses

The latest report from the U.S. Chemical Safety and Hazard Investigation Board has placed the blame on Pemex for a tragic accident at its Deer Park refinery in Texas back in October 2024. The incident involved a massive release of hydrogen sulfide gas that led to two fatalities among refinery workers.

According to the findings, the root cause was a missing standardized approach to identify idled equipment ready for maintenance. Workers mistakenly opened a flange on an active pipe while prepping nearby idle units, releasing about 27,000 pounds of this highly toxic gas.

Hydrogen sulfide levels measured during the incident soared beyond 500 parts per million, a concentration deadly to humans. One worker who handled the flange died instantly, while a second worker succumbed after being overcome by the drifting gas plume. Emergency services evacuated and treated thirteen additional employees exposed during the release.

The CSB pointed out that Pemex's existing protocols focused primarily on pipe marking for cutting but lacked explicit, standardized procedures for flange opening and blind removal - critical steps directly involved in this accident. The board suggested that a robust line marking system for all pipe openings could have averted the disaster.

The repercussions extended beyond the plant, prompting shelter-in-place orders in adjacent communities including Pasadena and Deer Park, highlighting the regional risks of petrochemical processing mishaps. Since the accident, Pemex reports it has implemented a comprehensive equipment identification system to prevent repeat errors.

No immediate comment was available from Pemex following the report's release. The Deer Park facility, a significant player in U.S. refining capacity, now faces the challenge of restoring confidence amid safety concerns raised publicly by a U.S. federal agency.

In the aftermath, the incident underlines the importance of procedural clarity in refinery operations where minute missteps can have fatal outcomes. With hydrogen sulfide's well-known hazards, industry observers wonder how similar gaps persist in large, complex facilities.
